This form is for use by licensees who need to provide a notification to the Department or request approval from the Department, as required under the regulations.

How to submit the form

The Form 27, along with any supporting documentation, must be submitted by the Designated Responsible Party (DRP) and must come from their registered email address.  The form should be sent to licensechange@cannabis.ca.gov.
