Depending on your selection, you will be required to submit either a notarized Form DCC 9205 Labor Peace Agreement statement or a copy of the signature page of your LPA during the document upload phase. Be prepared to provide the name of the labor organization and the LPA expiration date to ensure your renewal is processed smoothly.
